A proper survey requires questions. Presently we support six different styles of questions:

  • Checkboxes (Choose all that apply)
  • Multiple Choice (Choose one response)
  • Comment Box (Free text)
  • Star Rating (1 - 5 stars)
  • 5-Step Agree/Disagree (Likert Scale)
  • 7-Step Agree/Disagree (Likert Scale)

When entering questions we will display a preview that is indicative of what the learner will see when accessing the form.


Checkboxes Questions

A question that is of type Checkboxes allows you to create a question where the learner must choose one or more options in order to respond.


An example of a Checkbox question which asks the learner 'what they would like to do more' and has options to have offsite training, lunch meetings, incentive bonuses, and referral programs.

Multiple Choice Questions

A question that is of type Multiple Choice allows you to create a question where the learner may only choose one options from a list.


Example of a multiple choice question which asks the learner what snacks they like the least from a company lunch meeting and has the options of chips, pretzels, veggies, and cheese curls.

Comment box

A comment box is a free text box where a responder can say anything they would like.


An example of a comment box which asks the learner their thoughts on anything not already covered.

Star Rating

A star rating question allows the learner to choose between 1 and 5 stars on how they feel about the question.


An example of a star rating which asks how the learner would rate the training today.

5-Step Agree / Disagree

A 5-step agree / disagree question is a type of Weikart scale that allows up to 5 options. With these questions the learner will indicate their response by adjusting a slider. This slider has options to going from left to right:

  1. Strong disagree.
  2. Disagree.
  3. Neither agree nor disagree.
  4. Agree.
  5. Strongly agree.


An example of a 5-step agree / disagree form that asks the learner if they feel they are offering the best products to customers.

7-Step Agree / Disagree

A 7-step agree / disagree question is a type of Weikart scale that allows up to 7 options. With these questions the learner will indicate their response by adjusting a slider. This slider has options to going from left to right:

  1. Strong disagree.
  2. Disagree.
  3. Somewhat disagree.
  4. Neither agree nor disagree.
  5. Somewhat agree.
  6. Agree.
  7. Strongly agree.


An example of a 7-step agree / disagree form that asks the learner if they agree that they are providing the right level of service to clients.
